Initial ConsultationYour first appointment is a thorough evaluation of your shoulder pain and function. Our physicians perform a structured physical examination that includes range of motion assessment, targeted strength testing of each rotator cuff tendon, and provocative maneuvers designed to identify impingement and specific tendon involvement. Diagnostic ultrasound imaging of the rotator cuff tendons is performed at the initial visit to directly visualize tendon thickness, confirm the presence and characterize the size of any tear, assess for retraction, and evaluate the adjacent subacromial bursa. This imaging also guides the injection procedure if treatment is indicated at the same visit. You leave the first appointment with a confirmed diagnosis, an understanding of the tear's characteristics and prognosis, and a personalized treatment plan with clear goals and a realistic timeline.
Comfort and Preparation• The skin over the injection site is cleaned with an antiseptic solution before any procedure.
• A local anesthetic is applied to the skin at the needle entry point to minimize discomfort before the injection is placed.
• Your physician positions the ultrasound transducer to provide optimal visualization of the tendon and subacromial space and confirms your comfort before beginning.
Ultrasound-Guided Procedure Delivery• TThe ultrasound transducer provides real-time visualization of the rotator cuff tendons, the subacromial bursa, and the adjacent bone throughout the procedure.
• The needle is advanced under direct ultrasound visualization to the intended target, whether the subacromial space or the tendon defect itself, with position confirmed before injection is performed.
• For corticosteroid or PRP injection, the physician observes the spread of injectate in real time, confirming accurate delivery to the intended structure.
• The procedure takes approximately 15 to 20 minutes. Most patients tolerate it well with local anesthetic in place.
Immediate Recovery• Most patients return home the same day and resume normal light activities within 24 to 48 hours following the procedure.
• Mild soreness at the injection site is normal and typically resolves within one to two days. This soreness is distinct from the original shoulder pain and should not be interpreted as treatment failure.
• Apply ice to the shoulder for 15 to 20 minutes every two to three hours for the first day to manage post-procedure soreness.

• Implement the scapular and rotator cuff strengthening program consistently, not just during the acute phase of treatment. Returning to the same movement patterns and postural habits that contributed to the tear without correction increases the risk of tear progression.
• Patients receiving PRP therapy can expect gradual progressive improvement over eight to twelve weeks. A follow-up ultrasound at eight to twelve weeks can assess interval changes in the tear and guide further treatment decisions.

Treatment depends on the tear’s size, thickness, location, chronicity, and the condition of the tendon and muscle on imaging. Smaller tears may respond well to ultrasound-guided corticosteroid injections and targeted strengthening. PRP may be appropriate for select partial or small full-thickness tears. Larger or retracted tears, significant muscle atrophy, or tears that do not improve with conservative care may require surgical evaluation.

Many smaller partial-thickness tears and some small full-thickness tears can improve significantly without surgery. PRP may also support healing in appropriately selected tears. However, symptoms can improve even when a structural tear remains on imaging. Larger, retracted tears or those that fail conservative treatment are more likely to require surgical repair.

Rotator cuff tears can result from an acute injury, such as a fall, forceful lift, or shoulder dislocation, or from gradual tendon degeneration. Repetitive overhead activity, aging, smoking, diabetes, and family history can increase risk. PRP typically follows a slower healing process, with improvement developing over eight to twelve weeks. Some tears can enlarge over time, potentially causing increasing weakness and loss of shoulder function. Progressive tears may also lead to muscle atrophy and fatty changes, which can make later repair more difficult.
